  • New Mexico 2026 ACA enrollment up 20% y/y thanks to backfilling ALL lost federal tax credits; residents still have until MIDNIGHT TONIGHT for coverage starting TOMORROW!!

    BeWell NM, New Mexico's ACA exchange, is the only state marketplace with ongoing daily enrollment data reports:

    New Mexico Open Enrollment 2026 - Enrollment Summary

    Last Refreshed On: December 31st, 2025

    • Active Re-enrollees: 17,287
    • New Enrollees: 8,443
    • Auto-Re-enrollees: 53,042
    • Total: 78,772

    By comparison, at the same point last year, BeWell NM reported:

    • Active Re-enrollees: 19,216
    • New Enrollees: 6,644
    • Auto-Re-enrollees: 39,826
    • Total: 65,686

    In other words, new enrollments are up 27% while active renewals are down about 10%; they pretty much cancel each other out compared to the same point last year.

  • Maine: CoverME reports 30.9K active enrollments thru 12/27; 58.2K total so far

    via CoverME, Maine's ACA exchange:

    The Maine Department of Health and Human Services (DHHS) Office of the Health Insurance Marketplace (OHIM) will release biweekly updates on plan selections through CoverME.gov, Maine’s Health Insurance Marketplace.  

    Plan selections provide a snapshot of activity by new and returning consumers who have selected a plan for 2026. “Plan selections” become “enrollments” once consumers have paid their first monthly premium to begin coverage. These numbers are subject to change as consumers may modify or cancel plans after their initial selection.   

    The deadline to select a plan for coverage beginning January 1, 2026 is December 15, 2025. Consumers who select a plan between December 16, 2025 and January 15, 2026 will have coverage beginning February 1, 2026.

    TRENTON — With a critical deadline approaching to enroll in 2026 health coverage through Get Covered New Jersey, Department of Banking and Insurance Commissioner Justin Zimmerman is encouraging residents in need of health insurance to enroll by December 31st for coverage that is effective on January 1st . 

    Open Enrollment at Get Covered New Jersey, the state’s Official Health Insurance Marketplace, runs from November 1, 2025, through January 31, 2026, but consumers must enroll in a plan by December 31, 2025, for coverage beginning January 1, 2026. Those who enroll by January 31, 2026, will have coverage beginning on February 1, 2026.

  • California: Covered CA reports 617K active enrollments, up 1.3% y/y so far...but a deeper look portends disaster for many...

    via Covered California's Open Enrollment Dashboard, as of December 27th:

    • New enrollments: 137,958
    • Active renewals: 479,358
    • Passive Autorenewals: 1,318,779
    • Total: 1,936,095

    As I've noted before: While I include the passive/auto-renewal number for completeness sake, that number won't really be relevant until after the deadline for January 1st coverage passes (which isn't until New Year's Eve in California).

    More important for the moment is the total number of active enrollments, which includes both new enrollees as well as current enrollees who log into their account and actively select a plan for 2026.

    In California, those come to 617,316 combined.

    As of the same point last year (actually 1 day more; the data from last year is as of 12/28), Covered CA was reporting:

  • CMS posts claimed September 2025 Medicaid/CHIP enrollment report: 77.1M, down 2.4M since December 2024

    The Trump Regime has published an update to the official Medicaid/CHIP enrollment data:

    September 2025 Key Findings

    Medicaid and CHIP Enrollment

    • In September 2025, 77.1 million individuals were enrolled in Medicaid and CHIP.
    • 69.8 million individuals were enrolled in Medicaid, and 7.3 million individuals were enrolled in CHIP.
    • 40.3 million adults were enrolled in Medicaid, and there were 36.7 million Medicaid child and CHIP enrollees.

    Total Medicaid/CHIP enrollment in September 2025 of ~77.1 million dropped about 0.9% from July 2025, or by around 700,000 people.

    Since we're past the initial December 15th Open Enrollment Deadline for coverage starting January 1st in most states and since there's been several enrollment deadline extensions announced by various states (including two more today), I figured this would be a good time to post a completely updated rundown of just what the remaining deadlines are for every state as well as what your options are if you miss them.

    This is basically a standalone version of the first section of my annual Open Enrollment Guide; I hope that separating it out will make it less overwhelming.

    IDAHO: December 15th was the only deadline for 2026 coverage. If you missed it, you're pretty much out of luck unless you qualify for one of the following exceptions:

  • "But how many have PAID?" Revisited: Enrollment drop-off won't be fully known for months

    Long ago in a galaxy far, far away, during the first batcrap insane 2014 Open Enrollment Period, a common refrain from Republicans looking to attack the newly-ramping up healthcare law was to demand "BUT HOW MANY HAVE PAID???"

    The gist of the attack on the ACA was that after the worst of the ugly technical problems experienced by HealthCare.Gov and many of the state-based exchange websites were resolved, there was indeed a rush of backed-up demand to enroll in healthcare coverage by millions of people, but that just because a lot of people signed up for insurance coverage via the exchanges, that didn't necessarily mean they were actually enrolled in that policy.

    The reason for this, of course, is that you have to actually pay your first monthly premium in order to have your enrollment "effectuated" (i.e., put into effect).

  • California: Covered CA reports 584K active enrollments, up 2.8% y/y so far

    via Covered California's Open Enrollment Dashboard, as of December 20th:

    • New enrollments: 123,461
    • Active renewals: 460,670
    • Passive Autorenewals: 1,355,397
    • Total: 1,939,528

    As I've noted before: While I include the passive/auto-renewal number for completeness sake, that number won't really be relevant until after the deadline for January 1st coverage passes (which was December 15th in most states, but not until 12/31 in CA, IL, MD, NV, NJ, NM, PA, RI & VA).

    More important for the moment is the total number of active enrollments, which includes both new enrollees as well as current enrollees who log into their account and actively select a plan for 2026.

    In California, those come to 584,131 combined. This also means that only 25% of current enrollees had actively re-enrolled as of 12/20.

    As of the same point last year (actually 1 day more; the data from last year is as of 12/21), Covered CA was reporting:

  • CMS touts Nebraska being first to launch Big Ugly Bill Medicaid work reporting requirements by misspelling the name of the state.

    As of March 2025, around 72,000 Nebraska residents were enrolled in Medicaid via the ACA's expansion of the program.

    Last summer, after Congressional Republicans passed their Big Ugly Bill, I warned people against assuming that the gutting of Medicaid wouldn't hit until after the 2026 midterms:

    Over at The New Republic, Greg Sargent has taken this thinking one step further, noting that by delaying so much of the ugliness of the new law until 2027 or beyond...

    Republicans know how unpopular all this will be. So they’ve structured the bill so the tax cuts land immediately, while many of the Medicaid cuts get going in 2027 and 2028. That’s meant to spare them in the midterms.

  • Dr. Oz announces 15.6M HC.gov enrollees to date, down 600K from last year...but no CMS report

    Just before shutting down for Christmas, Trump Regime CMS Administrator Dr. Oz announced that...

    Quick ACA Update: More than 15.6 million Americans have enrolled in a plan on the federally run exchanges so far — compared to roughly 16 million Americans last year.

    Notably, this small drop follows several important CMS actions over the past year to combat fraudulent and improper enrollments, which have already removed more than enough people from premium subsidies who are covered elsewhere to account for the modest enrollment change. That said, there is a politically motivated lawsuit that has paused critical actions to make sure Biden-era improper enrollments are fully knocked out.

    IMPORTANT NOTE: For those who need insurance and have not already signed up — go to HealthCare.Gov to see if you qualify and what types of plans you can get. Most Americans can access a plan for $21 a month!

  • Colorado: Connect for Health CO announces 256K enrollees for Jan. 1st coverage

    via Connect for Health Colorado:

    Denver, Colo.– Enrollment for health coverage beginning Jan. 1 is consistent with this time last year, Connect for Health Colorado announced today. 256,422 residents have enrolled in a health insurance plan through the state’s official health insurance marketplace. That is approximately 0.14% higher than the number of people who enrolled for Jan. 1 coverage last year.

    Connect for Health Colorado is the only place where Coloradans can get financial help to purchase health insurance, and an estimated 65% of customers will qualify for financial help for plan year 2026. More than half – 58% – of customers who currently receive financial assistance will have at least one 2026 plan option with an estimated net premium under $10 per month.

  • California: CoveredCA encourages all residents to explore options before Dec. 31st deadline for Jan. 1st coverage

    Covered California is encouraging all uninsured Californians to sign up for health insurance by Dec. 31.

    The open-enrollment period for 2026 ends on Jan. 31, but to have insurance in place for January, consumers need to enroll by the end of the year. Covered California’s over 1.9 million enrollees also have until Dec. 31 to make any changes to their coverage for next year.

    It has been a unique open enrollment with the Enhanced Premium Tax Credits set to expire heading into 2026. Since their introduction in 2021, the enhanced assistance has helped double enrollment nationally under the Patient Protection and Affordable Care Act and by over 23 percent in California.

    So far this open enrollment, 123,461 Californians have signed up for 2026 coverage as of Dec. 20, marking a 30 percent decrease over the same period last year.

  • Golf Carts w/missing wheels: Rebutting Sen. Lankford's CNN Appearance, Part 2

    Yesterday I corrected a bunch of factual misstatements (aka "lies") and gross exaggerations about the ACA's premium tax credits and the GOP's refusal to extend the enhanced subsidy formula made by Oklahoma Senator James Lankford in his appearance on CNN's State of the Union with host Kasie Hunt.

    Today, I'm addressing the most critical exchange of his appearance:

    LANKFORD: "I would tell you uh healthcare in 2010 before Obamacare kicked in. Healthcare in 2010 the normal premium was $215. $215. Now take what it is now after Obamacare has been put in place and I think the..."

    HUNT: "...Right, but insurance companies could could refuse to cover you for a pre-existing condition. I mean the health care system was vastly different before Obamacare for for many of those reasons."

  • CMS posts claimed September 2025 Medicare enrollment; over 51% now enrolled in Med Advantage

    The Centers for Medicare & Medicaid Services just published updated enrollment data for Medicare, adding September 2025 to the data archive.

    Whether the data posted since January 20, 2025 is accurate or not, I can't say for certain, but at least they're updating it...and so far, at least, I don't see anything in their monthly reports which is setting off any obvious red flags.

    In any event, according to the latest report, as of September 2025:

    • Total Medicare beneficiaries are up to 69.36 million (up ~77K month over month)
    • Traditional/FFM Medicare beneficiaries are at 33.79 million (flat m/m)
    • Medicare Advantage beneficiaries reached 35.57 million (up ~77K m/m)
    • 12.12 million Medicare enrollees (around 17.5% of the total) were "Dual Eligibles"...that is, enrolled in both Medicare and Medicaid.

    The only number which seems odd is the 384K increase in Dual Eligibles...I'm not sure what to make of that.

  • CMS posts Medicaid expansion enrollment as of March 2025: Still over 20.7 million

    The Centers for Medicare & Medicaid Services (CMS) has posted an official update to the enrollment data for ACA Medicaid expansion through March 2025.

    According to the new report, total enrollment from December 2024 through March 2025 only dropped slightly (just over 30,000 people, or just 0.14%), and remained at over 20.7 million nationally, so it doesn't look like the Trump Admin has started cooking these particular books, at least not yet.

    I've been able to cobble together more recent ACA expansion enrollment for about half of the 40 states (+DC) which participate in the program; since March, expansion enrollment has dropped in some states but is actually up in some others. It remains remarkably stable so far, although this is going to change radically with the draconian provisions of the GOP's Big Ugly Bill starting to kick in soon.

  • Rebutting Sen. Lankford's Disingenuous CNN Appearance: Part 1 of 2

    Yesterday morning on CNN's "State of the Union," host Kasie Hunt talked to Oklahoma GOP Sen. James Lankford about the enhanced ACA tax credits which are currently scheduled to expire exactly 10 days from now.

    This gets into the weeds a bit, so I'm breaking it into two separate posts; I'll be publishing the second part tomorrow.

    The crux of the CNN appearance was Langford claiming that "Obamacare" (the Affordable Care Act...guys, he's been out of office for nearly 9 years now, let it go willya?) "caused prices to skyrocket in the marketplace" and that the expiration of the enhanced tax credits put into place in 2021 during the COVID pandemic is simply "exposing the real issues" within the ACA.

    First of all, let's clear up this "they were only put in place due to the COVID pandemic" talking point which Republicans keep tossing around (the implication being that since the COVID pandemic is over, the subsidy upgrade should end as well).

    Get it straight: Eliminating the 400% FPL subsidy cliff and beefing up the tax credit formula is something which Democrats always intended to do when they had the ability to do so.

  • Minnesota: 2nd chance for coverage: 2026 health insurance enrollment window remains open thru next month

    MNsure open enrollment ends January 15

    ST. PAUL, Minn. — Minnesotans have until January 15, 2026, to sign up for 2026 health insurance through MNsure. While the first enrollment deadline has passed, consumers can still go to the MNsure website to find a plan and sign up for coverage that starts February 1. The final deadline to enroll is 11:59 p.m. on Wednesday, January 15.

    Financial help for Minnesota health care plans is still available only through MNsure. MNsure-certified brokers and navigators provide free expert help and enrollment advice—from trusted professionals who won’t try to upsell consumers.

    “MNsure offers a variety of plans that meet Minnesotans’ needs and budgets,” said MNsure CEO Libby Caulum. “As the official health insurance marketplace, MNsure is the best place to shop for a plan you can trust, without fear of scams or false promises.”

  • Massachusetts: Residents have until Dec. 23 to find affordable, comprehensive health coverage for January 2026

    BOSTON—The Massachusetts Health Connector’s New Year deadline is looming, with residents without health coverage having until Dec. 23 to enroll in affordable, comprehensive health insurance that starts Jan. 1, 2026.

    Most people who apply for health insurance through the Health Connector get help paying for their coverage through the ConnectorCare program. The Health Connector is the state’s health insurance Marketplace, the only place where residents can get help paying for their coverage, and where residents can be assured they are getting a comprehensive health plan they can count on and avoid the junk plans that lurk on search engines.

    As I've noted before: While I include the passive/auto-renewal number for completeness sake, that number won't really be relevant until after the deadline for January 1st coverage passes (which was December 15th in most states, but not until 12/23 in MA & 12/31 in CA, IL, MD, NV, NJ, NM, PA, RI & VA).

    In California, those come to 535,349 combined. This also means that only 23% of current enrollees had actively re-enrolled as of 12/13.

  • Connecticut: Access Health CT announces deadline extension due to outage; 141K enrollees so far; emergency subsidies on the way

    Three important news items out of Connecticut today:

    First, via Access Health CT:

    HARTFORD, Conn. (December 16, 2025) — Access Health CT (AHCT), Connecticut’s official health insurance marketplace, experienced intermittent website outages on Monday, December 15, 2025 that impacted customers who wanted to enroll by that date to have coverage starting January 1, 2026. The website has been fully restored and is now available for eligible Connecticut residents to enroll in coverage.

    The Maine Department of Health and Human Services (DHHS) Office of the Health Insurance Marketplace (OHIM) will release biweekly updates on plan selections through CoverME.gov, Maine’s Health Insurance Marketplace.  

    Plan selections provide a snapshot of activity by new and returning consumers who have selected a plan for 2026. “Plan selections” become “enrollments” once consumers have paid their first monthly premium to begin coverage. These numbers are subject to change as consumers may modify or cancel plans after their initial selection.   

    The deadline to select a plan for coverage beginning January 1, 2026 is December 15, 2025. Consumers who select a plan between December 16, 2025 and January 15, 2026 will have coverage beginning February 1, 2026.

    ...Assuming new enrollment continues to only lag slightly behind last year, there could be up to ~4 million new enrollees for 2026, which means total plan selections could potentially hit ~25 million...which would actually break last year's record of 24.3 million.

  • DRAMA! In the House: 4 GOP members sign Dem tax credit discharge petition, forcing a vote...but...

    There's been a bunch of High Drama® within the House Republican caucus this morning as four "moderate" (aka, extremely vulnerable) Republicans have gone ahead and signed on to the Democrats discharge petition to force a House vote on a clean 3-year extension of the enhanced ACA tax credits.

    The four who signed on are Brian Fitzpatrick, Rob Bresnahan, Ryan Mackenzie (all of PA) & Mike Lawler of NY (see thread here).

    As of this writing, this development is also apparently causing a massive uproar, finger-pointing & backstabbing among House Republicans, with a lot of anger towards Speaker Mike Johnson, with at least one other vulnerable GOP member flat-out accusing him of lying, yadda yadda yadda.

    What this actually means in terms of the enhanced subsidies, however, is a different story.

    Welp. In a development which should surprise absolutely no one, GOP House Speaker Mike Johnson has announced that the bipartisan "Fitzpatrick Bill" which would include a 2-year extension of the enhanced ACA tax credits (albeit with significant caveats) won't be included in their healthcare bill vote this week after all.

    Speaker Mike Johnson confirmed Tuesday he will not allow a House vote this week to extend expiring Obamacare subsidies — a reversal from last week when a GOP leadership aide said the process “would allow” for an amendment vote.

    “In the end, there was not an agreement,” Johnson told reporters, noting the divides in his conference over the subsidies.

  • We're past December 15th. Now what? (Also: Extensions in IL & PA, and an important update in CT!)

    We're now over the hump: The initial deadline for people to enroll in ACA healthcare coverage starting on January 1st has passed in most states.

    HOWEVER, there's some important caveats to this, as well as some last-minute deadline extensions in a couple of states, so let's dig in...

    • If you live in IDAHO, last night was the one and only Open Enrollment Period deadline.

    This means that if you didn't actively select a plan for 2026, current enrollees are stuck with whatever plan Your Health Idaho automatically renewed theme into, while uninsured residents who didn't sign up by the deadline are mostly out of luck.

    The exceptions to this for both categories are a) if they're members of a federally-recognized Native American tribe (or are Alaska Natives); or b) if they are (or become) eligible for Medicaid or the Children's Health Insurance Program (CHIP). All of these are eligible to enroll year-round.

  • Reminder: The FINAL deadline for IDAHO residents to enroll is MIDNIGHT TONIGHT.

    In my annual ACA Open Enrollment Guide, the first piece of advice is always to make sure you don't miss the deadline to enroll in healthcare coverage for the upcoming year.

    I then list the various deadlines for people to get covered in each state, which is December 15th in most states for coverage starting January 1st (in a handful of states the deadline for January 1st coverage is as late as New Year's Eve).

    I always include the caveat, though, that if you do miss that deadline, you still have an extra month or so (January 15th in most states) to enroll for coverage starting February 1st. This means that at worst, you'll have to "go bare" for the month of January (and pray you don't get sick or injured for those 31 days) before your coverage kicks in.

    HOWEVER, there's an exception to this: IDAHO.

    In Idaho, residents only have until December 15th to enroll in ACA coverage for 2026, period.

    With the Senate failing to pass either the Democrats bill to simply extend the enhanced tax credits by 3 more years or the Republicans bill to make everything worse yesterday, all eyes turn to the House, where something like a dozen different bills, some with bipartisan support, some not, have been thrown against the wall to see if anything might stick.

    Now, it looks like one of them might actually get a floor vote next week...but only as an amendment to another healthcare bill package being pushed by GOP House leadership, which of course raises all sorts of red flags.

    As public health stakeholders and members of Congress grapple with how to respond to changes in HHS’ vaccine policy, the anti-vaccine organization founded by Secretary Robert F. Kennedy Jr., Children’s Health Defense, is petitioning FDA to remove mRNA COVID-19 vaccines from the market entirely.

    FDA has already signaled significant new scrutiny of the COVID-19 vaccines, with biologics center director Vinay Prasad telling staff the vaccines caused at least 10 pediatric deaths. So far, the agency hasn’t provided supporting data for that claim, but confirmed to Bloomberg it is investigating vaccine-associated deaths in both children and adults.

    OLYMPIA, Wash. – Today, the Senate failed to pass either of two health care bills they voted on, one of which was to extend the enhanced premium tax credits. Washington Health Benefit Exchange released the following statement:

    “We are deeply disappointed Congress failed to extend the enhanced premium tax credits (ePTCs). We know these tax credits have made a tremendous difference to Washingtonians. They have helped to make health insurance more affordable, and without them more people will be forced to go without coverage and increase the uninsured rate. We also know that many of our customers will be forced to make tough decisions to afford the increased costs.  

    “While this extension was not successful, we are grateful to members of our state Congressional delegation for their leadership in fighting for Washingtonians and highlighting this very important issue and the focus on health care affordability.

  • Breaking: Thing everyone expected to happen, happens. Now what?

    So, as expected, both the Senate Democrats bill to simply extend the enhanced ACA tax credits for 3 more years and the Senate Republican bill to make everything worse just failed to reach cloture (appropriately enough, they both received the exact same vote counts: 51-48).

    Sen. Rand Paul voted against the GOP bill and 4 Republicans voting for the Democrat's bill, including Sen. Josh Hawley, Sen. Lisa Murkowski, Sen. Dan Sullivan & Sen. Susan Collins voting for it. I'm not sure who the missing vote in each was, not that it matters much now.

    So, now what?

    Well, the House may end up voting on one of their dozen or so bills they've been tossing around, but I wouldn't count on it, and even if one of them passes, it'd be almost certain to fail in the Senate.

    With just 4 more days before the initial December 15th deadline to enroll for coverage starting in January, only a weeks' worth of legislative session days before the holiday break, and just 20 days before the enhanced subsidies actually expire on New Year's Eve, it's looking grim.

    OK, I know I said I wasn't gonna do a deep dive into any more last-ditch GOP bills ahead of tomorrow's big Senate vote, but this one looks intriguing...but not just for the reasons you might think.

    Earlier today, GOP Rep. Jen Kiggans & Democratic Rep. Josh Gottheimer introduced Yet Another short-term enhanced ACA tax credit extension bill...but this one has some very interesting twists.

    House Republican leaders presented no firm plan Tuesday for advancing health care legislation as anxiety rises in the GOP ranks over the impending expiration of key Obamacare subsidies at the end of the year.

    Instead, Speaker Mike Johnson presented attendees of a closed-door conference meeting with list of 10 possible policies that could get votes in the coming weeks or months, according to five Republicans in the room.

    Some were more specific, such as an expansion of health savings accounts and an overhaul of pharmaceutical benefit manager oversight. Others were vague, including one bullet point that simply said, “Innovation.” The list did not include an extension of the expiring tax credits.

  • California: Covered CA reports 451K active enrollments, up 4% y/y so far

    via Covered California's Open Enrollment Dashboard, as of December 6th:

    • New enrollments: 62,247
    • Active renewals: 389,329
    • Passive Autorenewals: 1,363,264
    • Total: 1,814,840

    As I've noted before: While I include the passive/auto-renewal number for completeness sake, that number won't really be relevant until after the deadline for January 1st coverage passes (which is December 15th in most states, although not until 12/23 in MA & 12/31 in MD, NV, NJ, NM & RI).

    More important for the moment is the total number of active enrollments, which includes both new enrollees as well as current enrollees who log into their account and actively select a plan for 2026.

    In California, those come to 451,576 combined. This also means that only 22% of current enrollees had actively re-enrolled as of 12/06.

    As of the same point last year (actually 1 day more; the data from last year is as of 12/07), Covered CA was reporting:

Even though enhanced premium tax credits for next year may not be extended through Congress, many customers can still find plans with state and federal savings.

Customers who enroll Dec. 16, 2025 through Jan. 15, 2026 will have coverage beginning Feb. 1, 2026.

    The Open Enrollment Period for Connecticut residents to shop, compare and enroll in health insurance or renew their coverage began Nov. 1, 2025 and runs through Jan. 15, 2026. Customers may enroll in health and dental coverage.

    Hot on the heels of Republican Senators Bill Cassidy of Louisiana & Mike Crapo of Idaho presenting their so-called "Health Care Freedom for Patients Act" which would do almost nothing to actually help ACA enrollees while causing more harm to transgender individuals (of course) and undocumented immigrants (of course), at least two more "The clock is ticking, we have to come up with SOMETHING!" bills have been rolled out today by Congressional Republicans.

    First up is this one from former centrist Democrat-turned-MAGA-Republican Congressman Jeff Van Drew of New Jersey (h/t Andrew Sprung for bringing it to my attention):

    Washington, DC -Today, Congressman Van Drew joined a bipartisan coalition of House members calling for a short-term extension of the Affordable Care Act's (ACA) tax credits with targeted modifications of the system. The group also sent a letter to House and Senate leadership asking for a meeting to discuss a clear path forward for health insurance.

    Scoop: Senate GOP chairs circulate health plan as clock ticks on ACA subsidies

    Two key Senate Republican chairmen are circulating the outline of a health care plan to Republican offices ahead of a crucial week that could decide the fate of enhanced Affordable Care Act subsidies, multiple sources tell Axios.

    Why it matters: The plan from Finance Committee chair Mike Crapo (R-Idaho) and health committee chair Bill Cassidy (R-La.) does not extend the tax credits past their expiration at year's end, instead providing funds to enrollees in health savings accounts, according to a summary obtained by Axios.

    It's not the only GOP plan that's been floated in recent days as Republicans struggle to unite around health care policies.

    TRENTON — In a bulletin issued today, the New Jersey Department of Banking and Insurance issued guidance to insurance carriers on health insurance coverage for childhood immunizations.

    The Department advises that, regardless of federal actions relating to the childhood immunization schedule, it expects carriers to provide coverage for childhood immunizations recommended by the New Jersey Department of Health, including for the hepatitis B vaccination series, in accordance with State law and without cost sharing or the imposition of additional barriers.

    We are aware of several provider network inaccuracies in our Plan Comparison Tool. The inaccuracies are related to Anthem plans. 

    The network issues related to Anthem plans are limited to Northern Light providers. We expect the Anthem network to be corrected by December 16. In the interim, consumers may use Anthem's Provider Lookup Tool (external to CoverME.gov).

    Oy...December 16th? That's the day after the initial Open Enrollment Deadline passes. Sheesh.

    For nearly a year now, I've been writing about the upcoming expiration of the enhanced ACA premium tax credits (eAPTC) which have been in place for the past five years. They're currently scheduled to expire at the end of December, less than 2 months from today.

    I spent most of the year posting projected estimates of how much more different households will have to pay.

    More recently, as the actual 2026 premium rate filings have been published, I revised those estimates with more accurate data.

    Over the past few weeks as the various ACA exchange websites brought their 2026 ACA window shopping live, I've started plugging in different household scenarios to see what actual, real world price hikes look like.

    In yesterday's write-up about CMS posting the first official, state-by-state data breakout for the ongoing 2026 ACA Open Enrollment Period, I noted several possible reasons why so far enrollment is running a solid 11% ahead of the same point a year ago even with widespread knowledge of the enhanced federal tax credits expiring just 25 days from now:

    • Total 2025 enrollment was 13.4% higher than total 2024 enrollment; assuming a similar rate of net attrition over the course of the year, there should be around 13% more current enrollees who can potentially actively renew/re-enroll for 2026 to begin with. As it happens, re-enrollment is currently up around 14% over the same point a year ago.

  • Breaking: CMS posts claimed 2026 Open Enrollment: 5.76M QHPs, up ~11% y/y so far

    Just before the close of business for the week, the Centers for Medicare & Medicaid Services (CMS) published the first official 2026 Open Enrollment Period snapshot report:

    The Centers for Medicare & Medicaid Services (CMS) reports that nearly 950,000 consumers who do not currently have health care coverage through plans in the individual market Marketplace have signed up for coverage in 2026, since the start of the Marketplace Open Enrollment Period (OEP) on November 1, 2025. Existing consumers are also returning to the Marketplace to actively renew their coverage, and anyone who does not actively renew will be automatically re-enrolled for 2026. Over 4.8 million existing consumers have already returned to the Marketplace to select a plan for 2026.

  • 38 under 15: ACA enrollment & looming premium hikes for every vulnerable GOP House member

    There are 43 U.S. House districts where the Republican nominee won by 15 points or less. Of those, one (WA-04) doesn't really count since there were 2 Republicans running in the general election (Washington State has "jungle primaries"). Four others were won by Donald Trump by between 16 - 20 points (AZ-08, CO-04, TX-15 & WI-08).

    That leaves 38 GOP-held House seats where the Republican won by 15 pts or less and where either Kamala Harris won, or Donald Trump also won by 15 points or less. The table below breaks these out with both margins, while also listing my estimate of how many residents of each district are enrolled in ACA coverage.

  • The Graph Returns...sort of.

    For 12 years now, one of the traditions of ACA Signups has been The Graph: A line graph tracking enrollment in ACA policies over the course of each Open Enrollment Period.

    The original Graph from 2013-2014 looked quite different from more recent years, partly because I was attempting to track Medicaid/CHIP enrollment and other coverage categories at the same time, and partly because, frankly, I didn't really know what I was doing at the time.

    Over the next few years I modified & improved both my methodology as well as the format, culminating in last year's 2025 Open Enrollment Period Graph, which featured the highest enrollment figures in the ACA's history: ~24.3 million Qualified Health Plan (QHP) enrollees, plus another ~1.8 million Basic Health Plan (BHP) enrollees in Minnesota, New York & Oregon specifically, for a grand total of just a hair over 26 million people.

    For the 2026 Open Enrollment Period, however, actual hard enrollment data has been, shall we say, difficult to come by so far.

  • California: Covered CA reports 410K active enrollments, either up 4.8% or down 7.9% y/y depending on your POV...

    I don't know if this is new or not, but it turns out that Covered California--the largest state-based ACA exchange in the country--has an Open Enrollment Dashboard after all!

    This means that in addition to two small states regularly reporting Open Enrollment data (Maine and New Mexico), the largest one is as well!

    Here's what Covered CA is reporting as of November 29th:

    • New enrollments: 45,023
    • Active renewals: 365,879
    • Passive/Autorenewals: 1,412,526
    • Total: 1,823,428

    As I've noted in both my Maine and New Mexico updates, while I include the passive/auto-renewal number for completeness sake, that number won't really be relevant until after the deadline for January 1st coverage passes (which is December 15th in most states, although not until 12/23 in MA & 12/31 in MD, NV, NJ, NM & RI).4,

  • Minnesota: MNsure Announces SEP for Strategic Limited Partners Policyholders

    Minnesota residents affected by the end of Strategic Limited Partners coverage can enroll in new health insurance through MNsure

    ST. PAUL, Minn.—MNsure is opening a limited special enrollment period (SEP) for Minnesota residents who purchased insurance through Strategic Limited Partners offsite of the MNsure website. The SEP follows enforcement action by the Minnesota Department of Commerce.

    Commerce found that Strategic Limited Partners, an unlicensed company, sold unauthorized and deceptive health coverage to Minnesotans through misleading ads. Under a consent order, Strategic Limited Partners must cease operations by December 31, 2025, notify customers of its exit, repay outstanding claims, and pay a civil penalty.

    To protect affected consumers, MNsure is offering this limited SEP:

  • New Mexico's amazing emergency ACA policies take it ANOTHER step further...

    Back in October, I noted that the New Mexico Insurance Superintendent--in the midst of dropping some very bad news about the upcoming 36% average premium increases for New Mexico ACA policies--included an extremely important caveat:

    While it appears that Congress will allow enhanced federal Premium Tax Credits to expire, New Mexico’s Health Care Affordability Fund (HCAF) will cover the loss of the enhanced premium tax credits for households with income under 400% of the Federal Poverty Level (or $128,600 for a family of four), providing up to $68 million in premium relief for working families who enroll in coverage through BeWell in 2026. Federal and state premium assistance will continue to reduce the impact of the rate increases.

  • Maine: CoverME reports 14.5K active enrollments in November; 57.9K total so far

    CoverME, Maine's ACA exchange, is the second state after New Mexico to publish official 2026 Open Enrollment data:

    The Maine Department of Health and Human Services (DHHS) Office of the Health Insurance Marketplace (OHIM) will release biweekly updates on plan selections through CoverME.gov, Maine’s Health Insurance Marketplace.  

    Plan selections provide a snapshot of activity by new and returning consumers who have selected a plan for 2026. “Plan selections” become “enrollments” once consumers have paid their first monthly premium to begin coverage. These numbers are subject to change as consumers may modify or cancel plans after their initial selection.   

    The deadline to select a plan for coverage beginning January 1, 2026 is December 15, 2025. Consumers who select a plan between December 16, 2025 and January 15, 2026 will have coverage beginning February 1, 2026.

    BeWell NM, New Mexico's ACA exchange, is the only state marketplace with ongoing daily enrollment data reports:

    Officially, they're reporting 75,926 Qualified Health Plan (QHP) enrollments already, which is actually 8% higher than the 70,373 which they ended with during the 2025 Open Enrollment Period (OEP) last January.

    HOWEVER...and this is a major caveat...that 75,926 includes all current enrollees being auto-renewed for 2026, which doesn't really count for my purposes. Most state exchanges used to hold off on lumping in the auto-renewals until after the initial December deadline, only reporting current enrollees who actively re-enroll along with new enrollees.

    When I look at it that way, I get:

    For nearly a year now I've been shouting from the rooftops about the eye-popping net premium hikes which millions of ACA enrollees are going to see starting one month from today, assuming the enhanced Advanced Premium Tax Credits (eAPTC) which have been in place for the past five years are allowed to expire on New Year's Eve.

    I've put together 51 bar graphs showing examples of what these net premium increases will look like for various households at different income levels in every state. Since there's so many variables from state to state including different Rating Areas, different levels of carrier participation, different provider networks and different benchmark Silver plans from county to county (and even from zip code to zip code), I decided to use the capital city of each state as my rule of thumb.

    For the households, I went with four case studies: A single 50-yr old adult w/no dependents; a 30-yr old single parent with one child; a "nuclear family" (40-yr old couple with two kids age 15 & 12); and a pre-retiree couple (64 yrs old, just shy of Medicare eligibility age).
